"婦人科領域における腹腔鏡手術の普及はめざましく，患者からの要望も増加している．そのため当院では卵巣腫瘍に関しては，悪性が強く疑われる場合を除き腹腔鏡手術を選択している．平成17年1月から平成19年7月までの期間に腹腔鏡下手術を行った卵巣腫瘍140例のうち，術後診断で境界悪性と診断された症例が3例あり，いずれも卵巣の保存手術を希望していた．このうち2例は粘液性境界悪性で，1例は片側付属器摘出と虫垂切除，1例は嚢腫摘出術と虫垂切除が施行された．1例は漿液性境界悪性で術前より悪性の可能性も疑われたため，組織検査をかねて片側付属器摘出を行った．いずれの症例も腹腔内への進展は認めず，追加手術は行わなかった．これらの症例は化学療法も行わず経過観察している．術後5～11ヶ月間経過しているが，現在のところ再発を認めていない．このように術中または術後に境界悪性と診断される症例が存在し，再手術の選択や治療方針に苦慮することが多い．これらの3症例をもとに，境界悪性の取り扱いについて文献的考察を加え，当院における卵巣の保存を目的とした腹腔鏡下卵巣腫瘍手術の基本方針をまとめた．",
