Center","bibliographic_titleLang":"en"}]}]},"item_10001_description_5":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"糖尿病性ケトアシドーシスはインスリン欠乏状態に起因して高血糖,脱水,代謝性アシドーシスをきたす病態であり,高ケトン血症を呈することが特徴的であるが,血中ケトン体濃度は通常診断基準としては用いられず,その臨床的意義を検討した報告も少ない.種々の重症度の糖尿病症例で血中ケトン体濃度を血液ガス分析のデータと比較検討することで,糖尿病の重症度判断における位置づけを考察した.総ケトン体,アセト酢酸,βヒドロキシ酪酸の血中濃度はいずれもpHおよびHCO3-と負の相関を示し,pH7.3・HCO3- 18mmol/Lに相当するβヒドロキシ酪酸血中濃度はそれぞれ4565.8µmol/L,4624.4µmol/Lであった.またβヒドロキシ酪酸血中濃度のカットオフ値推定のためROC曲線より感度-(1-特異度)が最大となる点を求めると,DKAの診断では6260µmol/L(感度81.5%,特異度100%),ICUへの入院は7379µmol/L(感度90%,特異度87.1%),インスリンポンプを要した水準は6260µmol/L(感度79.2%,特異度93.7%)であった.尚,上記いずれも感度特異度ともにHCO3-には及ばなかった.血中ケトン体濃度は糖尿病性ケトアシドーシスの診断,重症度の判断において有用であるが,既存の診断基準に対する優位性は見出せず,重症糖尿病の診療における血液ガス分析の重要性は揺るぎないと考えられる.","subitem_description_type":"Abstract"}]},"item_10001_full_name_3":{"attribute_name":"著者別名","attribute_value_mlt":[{"nameIdentifiers":[{}],"names":[{"name":"Hirobata,
