{"created":"2023-06-20T15:57:00.780576+00:00","id":14140,"links":{},"metadata":{"_buckets":{"deposit":"84275b2d-462a-4b0c-8a8d-98ba79db92eb"},"_deposit":{"created_by":18,"id":"14140","owners":[18],"pid":{"revision_id":0,"type":"depid","value":"14140"},"status":"published"},"_oai":{"id":"oai:redcross.repo.nii.ac.jp:00014140","sets":["3:560"]},"author_link":["51293","51291","51294","51289","51290","51292"],"item_10001_biblio_info_7":{"attribute_name":"書誌情報","attribute_value_mlt":[{"bibliographicIssueDates":{"bibliographicIssueDate":"2018-09-20","bibliographicIssueDateType":"Issued"},"bibliographicIssueNumber":"1","bibliographicPageEnd":"349","bibliographicPageStart":"346","bibliographicVolumeNumber":"69","bibliographic_titles":[{"bibliographic_title":"日赤医学"}]}]},"item_10001_description_5":{"attribute_name":"抄録","attribute_value_mlt":[{"subitem_description":"【目的】Ａ病院の平成28年度全入院患者の高齢者の占める割合は62％であり、そのうち60.6％が高齢者世帯、家族の就労による介護力不足など退院困難な要因を有し、退院支援対象患者の入院期間延長の原因となっている。病院経営にとって、退院支援困難な要因を持つ患者の円滑な退院支援体制を構築することは重要であると考える。そこで退院支援加算１体制の定着を目的、地域医療連携課退院支援係が中心となり、院内・院外の連携を強化することで、退院支援の実施率をアップすることができたため、そのプロセスを報告する。【方法】7つの取り組みを実施した。1)各病棟へのMSW配置による退院支援体制の充実。2)退院支援における院内多職種の役割と協力体制を明確化し、チームで取り組む体制を作る。3)退院支援体制の再構築を病院戦略目標活動の一つとして、全組織的な取り組みを行う。4)病棟看護師の退院支援推進者となる退院支援院内認定看護師の育成。5)連携保健医療機関との連携実務担当者による情報交換会の開催（データベース作成）6)在宅サービス担当者との情報交換会の開催（訪問看護ステーション、介護支援専門員との連携強化）7)医療・介護・患者家族連携による入院早期（7日以内）の退院支援計画協働立案システムの構築。平成27年度の退院調整加算体制と平成28年度の退院支援加算体制１の実績を比較した。【結果】退院支援実施率30.6％から50.9％（20.3％増）、算定率20.5％から41.5％（21.0％増）、退院支援患者の在宅復帰率78.1％から88.6％、平均在院日数24.7％から18.9％となり退院支援に関する稼動額が増加した。【結論】院内・院外の連携を強化して、全組織的な取り組みと地域を巻き込んだ活動は有効である。","subitem_description_type":"Abstract"}]},"item_10001_full_name_3":{"attribute_name":"著者別名","attribute_value_mlt":[{"nameIdentifiers":[{}],"names":[{"name":"NISHIMUKAI,
